Michelle Forman, PT, DPT
Physical Therapist
Michelle Forman, PT, DPT has extensive experience in the diagnosis and treatment of musculoskeletal and neurological movement conditions. Michelle completed a Bachelor of Science in Health Ecology at the University of Nevada, Reno followed by a Doctorate in Physical Therapy at Samuel Merritt University. She has practiced as a physical therapist since 2010 in the inpatient, acute rehabilitation, and outpatient settings. Michelle has previously obtained her certificate in neuro- developmental treatment, BIG program for patients with Parkinsons, and recently completed continuing education in spinal cord injury rehabilitation.
Michelle is actively involved in performing physical therapy evaluations, establishing patient plans of care, establishing home exercise programs, and providing follow-up communication with referring practitioners on patient progress. Michelle develops physical therapy plans for patients with both acute and chronic neurological, orthopedic, debility, amputation, and balance impairments. She works with the non-surgical physical medicine, pain management, radiology and neurosurgeon specialists at SpineNevada (in Reno and Sparks) to establish comprehensive treatment plans with the goal of helping each patient improve function and relieve painful symptoms.
Michelle enjoys helping each patient reach their individual goals for return to optimal function and activity.
